Care Management-A mutual agreement between the identified client and the firm to ensure necessary needs are identified, addressed, and ultimately resolved. Care Management could be a less restrictive alternative to guardianship and a way to add additional support and oversight to an individual and their day to day living. This is also an excellent alternative for families, who still need/want their loved one looked after but cannot do so on their own due to their own time constraints, distance, or other commitments.

Discovery of Needs/Services-Utilizing a multi-disciplinary approach to fact gathering, the discovery of needs and services is critical to a successful Plan of Care.

Plan of Care-A process in which a plan is created to aid in the most effective and successful outcome for the individual. This process also will direct what supports and services need to be employed to ensure productive results.

Research/Coordination of Services and Supports- Many times there are additional resources, programs, and services that a loved one may need. Through the Social Service program at Hammerle Finley Law Firm, the highly qualified staff will research, and walk with the individual to access those needed resources.
